The story focuses on the state of the world 28 years after the Rage Virus outbreak devastated the population of England. A number of survivors have managed to build a community on a small island that is under strict quarantine, connected to the mainland by a single strong defensive route.

When a young boy tries to find a doctor to save his dying mother, he is forced to leave the island and cross a bridge to the mainland controlled by another group of survivors and mutant infected. Along the way, the boy faces not only the threat of agile zombies that have mutated but also conflicts among survivors with different survival instincts.

Throughout the story, they uncover horrifying secrets about the mutation of the virus and the dark side of humanity that has persisted nearly three decades after the outbreak first occurred.

The main cast of 28 Years Later includes Jodie Comer as Isla, a woman who suffers from memory loss and lives with her family on the quarantine island of Lindisfarne, Northumberland; Aaron Taylor-Johnson as Jamie, Isla's husband who acts as a scavenger for their community, and Ralph Fiennes as Dr. Kelson, a survivor trying to uncover and cope with the long-term effects of the Rage Virus.

Jack O’Connell stars as Sir Jimmy Crystal, a cult leader with a dark past who has a strong influence on the mainland, while Alfie Williams plays Spike, a 12-year-old boy who becomes a key figure in the mission to save his mother. Erin Kellyman portrays Jimmy Ink, a mysterious character with questionable loyalty, and Edvin Ryding appears as Erik Sundqvist, a Swedish soldier who is part of NATO North Sea Patrol and stranded in northern England. Chi Lewis-Parry appears as the Alpha, the physical leader of the infected who have mutated, while Emma Laird plays Jimmima, one of the survivors trying to rebuild hope for her community.
