According to police statements, the fatal incident occurred in the early hours, specifically on Saturday around 00:30 WIB. The location of the incident was on Jalan Kesehatan Raya, South Jakarta, which at that time was reported to be quite quiet. Gary Iskak was known to be riding a motorcycle alone when the accident happened.

Based on initial reports gathered from eyewitnesses at the scene of the incident (TKP), Gary allegedly lost control of the two-wheeled vehicle he was riding. The motorcycle then swerved and crashed into a tree that was right in front of a car wash. The hard impact is believed to have caused the fatal injuries suffered by the actor.

Although he received medical attention, fate had other plans for Gary Iskak. His life could not be saved after the incident, and medical personnel stated that he took his last breath on Saturday morning at 09:24 WIB.
